To support your application, you’ll need to submit documents based on the type of services or products you provide:

For Providers Selling Products:

• A valid Business License, including:
  • Articles of Incorporation/Organization and/or
  • IRS document

For Providers Offering Services:

• A valid Business License, including:
  • Articles of Incorporation/Organization and/or
  • IRS document
• Professional License Numbers and Documents
